I authorise Coaches to give permission for me to receive medication and any emergency dental, medical or surgical treatment, including anesthetic, as considered necessary by medical authorities present.

I agree that photographs/videos may be taken on behalf of Irish Water Safety Clare at events in line with IWS Code of Ethics and Good Practice and GDPR policy.

I agree to abide by the rules of competition and the rules of Irish Water Safety Clare and Irish Water Safety.

I agree to compete for WSI Clare SLS at competitions having attended the required training.

Parental consent (if child)

I authorise Coaches or other adults with parental responsibility to give permission for my child to receive medication and any emergency dental, medical or surgical treatment, including anesthetic, as considered necessary by medical authorities present.

I/my child agrees that photographs/videos may be taken on behalf of Irish Water Safety Clare at events in line with IWS Code of Ethics and Good Practice for Children involved in Water Safety Activities in line with GDPR policy.

I/my child agrees to be respectful of coaches, helpers and fellow competitors, to abide by the rules of competition and the rules of Irish Water Safety Clare and Irish Water Safety.

I agree to carry out chaperone duties as required.

I/my child agrees to compete for WSI Clare SLS at competitions having attended the required training.
